So LDAP lookup is working. I can't figure out what I'm missing. If your username is not on that page then you've configured something incorrectly in your webserver and its not authenticating the users properly. If you do see your username there can you copy and paste the line or lines that its on and paste it here and i'll see if I can work out what is going wrong with things.
I also don't see where I would find it. What I find worrying is that in the administrator section the phpinfo doesn't come up with the username. As the administrator phpinfo is exactly that, a phpinfo call, with different CSS. So if you don't see it there then the directory is probably not being protected properly you should protect your Joomla!
Have a play in IIS to see if you can get things working properly. This seems to be a known issue with J! Auth - Joomla enabled - Second in order.
Still, much strangeness I guess access would be good and I can see how things work. I'm having the exact same issue. I think there was an issue with the username replacement, I haven't gotten around to releasing the update yet because well I just haven't had the time to put all of the little dependencies together. If you use something like Firefox not configured properly it should prompt you with a password, if this isn't happening then more than likely you don't have something set up properly. I'm not an IIS expert, I only dabble in it. Have a try with different things and combinations of settings but in my limited experience all I've had to do is select the Windows authentication and I'm off.

When I have the time, I quite literally build up a VM to do a whole heap of Windows testing with one of their eval copies and then let it rot and expire. Did you manage to fix it?
